In this prospective multicenter study conducted in hospitals participating in the Dutch Thyroid Network, researchers investigated whether measuring parathyroid hormone (PTH) levels on the first day after total or completion thyroidectomy can predict which patients will develop persistent hypoparathyroidism. After one year, 17% of patients still required calcium and/or active vitamin D supplementation. All of these patients had experienced a >70% drop in PTH compared to their preoperative baseline.

Among patients without such a PTH decrease, none developed persistent hypoparathyroidism. The predictive value was high, with a sensitivity of 100% and specificity of 73%. This measurement provides a simple tool to identify high-risk patients early and offer them appropriate follow-up care.
