This multicenter study shows that the incidence of hypoparathyroidism strongly depends on how it is defined.

This retrospective multicenter study in the Southwest Netherlands evaluated the incidence of persistent hypoparathyroidism following total thyroidectomy. Researchers compared three common definitions with a newly proposed pragmatic definition. Depending on the definition used, the reported incidence ranged from 7.9% to as high as 22.1%.

The findings underscore the lack of international consensus and how that contributes to wide variability in reported rates. The newly proposed definition — based on continued use of active vitamin D and a documented failed attempt to taper it — may allow for better international comparison across studies. Risk factors identified for persistent hypoparathyroidism included low postoperative calcium, parathyroid autotransplantation, and the presence of other surgical complications.
